Overview
This article covers the three main edits support agents can make to integrated medical history forms using Tools: changing the form type (radio vs. dropdown), hiding or showing individual medical alerts, and regenerating the form after a practice updates its EHR alerts.
What you cannot change
Before making any edits, note these hard limits — they apply to all EHRs except where noted:
The text of individual alerts (conditions, allergies, medications) must be changed in the EHR itself. NexHealth reads them exactly as they appear there.
You cannot correct spellings, capitalization, or verbiage on synced alerts.
You cannot add new alert items in NexHealth — only the EHR can add them.
Eaglesoft is a special case: no edits of any kind can be made to the Eaglesoft medical history form in NexHealth. All changes must happen in Eaglesoft directly.
Change the medical history form type (radio vs. dropdown)
Applies to: all EHRs except Eaglesoft
In Tools, go to Forms Setup.
Find the location you want to update and click its Form Settings gear.
In the Form Settings window, find the Medical History form type dropdown.
Select either Radio button or Dropdown.
Click Save.
Radio button — presents patients with a yes/no list of all alerts from the EHR.
Dropdown — presents a search box; patients type to find and select their alert. Any item typed that doesn't match an existing alert lives only on the PDF and does not sync back to the EHR.
Hide or show individual alerts
Applies to: all EHRs except Eaglesoft; radio button format only
You can hide misspelled or unwanted alerts so they don't appear on the patient-facing form. Hidden alerts may still be visible in the Form Builder edit mode, but patients will not see them.
This option does not apply to the Dropdown format — alerts in that format populate based on what the patient types, so there is no list to hide items from.
In Tools, navigate to the location's Forms Setup page.
Click Edit Synced Medical Alerts.
Toggle off any alerts you want to hide from the patient form.
Save your changes.
Regenerate the medical history form
Use this when a practice has added or changed alerts in their EHR and the form hasn't updated in NexHealth.
Standard regeneration (most EHRs)
In Tools, go to Forms Setup for the location.
Click Enlive Sync.
Select Medical History Form.
A green confirmation box will appear when the new form has been generated.
eCW, Athena, Cloud9, ModMed, NextGen Office — alerts not appearing after install
If alerts didn't read in correctly, run sync commands first, then regenerate:
In Tools, click Create Sync Commands in the Production menu.
Select Single at the top.
Choose the Institution and Location.
From the Sync Command Type dropdown, run a separate sync command for each of the four commands that begin with "Medical."
Click Apply, then Create Command for each.
Monitor progress on the Sync Commands page in Tools. All four should reach a green Complete status.
Check Datadog for errors. If you see
No 'alert type' records returned by the EHR, the practice has no alerts of that type in their EHR — this is expected.Once all commands complete, go to Forms Setup → Enlive Sync → Medical History Form to regenerate.
Dentrix Ascend — alerts updated after initial install
Dentrix Ascend reads medical alert categories via the Chrome extension. Use this method if a practice has changed their alerts in DxA after onboarding:
Ensure the Chrome extension is logged in.
Log in to Dentrix Ascend.
Navigate to
<base_URL>/bootstrap/medicalAlertCategory(e.g., https://live2.dentrixascend.com/bootstrap/medicalAlertCategory). The extension will read the updated alert categories.Regenerate the form: Tools → Forms Setup → Enlive Sync → Medical History Form.
Once regenerated, update the Document Type to ensure the form routes to the correct folder in DxA.
